Questions people ask
Is the typed name a legally binding signature?
No. The form records a typed name and a date, not a legally binding e-signature. Check what your situation requires before relying on it.
How do I add my own consent wording?
Using the template creates a draft you can change completely. Replace the statement on the second screen with your own wording, then add anything else you need, such as a question for a child’s name.
What happens if someone says no?
The form ends there with a message that you can’t go ahead without their consent. You can change that message, or the rule, in the editor.
Where are the consent records kept?
Every submission lands in your form’s Responses tab, and you can download them as a CSV for your records.
