Questions people ask
Can I change the rating areas?
Yes. Use the template and it becomes a draft you can change completely: rename the areas, switch the scale labels, add your company values, or delete what you don’t use.
What do the ratings mean?
Each area is rated 1–5, from “Below expectations” to “Exceeds expectations”. Most teams treat 3 as fully meeting expectations. Agree what each point means with other managers so ratings compare fairly across teams.
Can the employee add their side?
This form is filled in by the reviewer. Pair it with the Self evaluation form so the employee shares their view before you meet.
Can it be signed?
Legally binding e-signatures aren’t available. If you need a signed copy, download the responses as a CSV and handle sign-off the way you do today.
